To be eligible for a host home a person must have a Medicaid Waiver that is funded for residential supports. An application is completed by the person and along with supporting documentation it is sent to the Georgia Department of Behavioral Health and Developmental Disabilities Regional Field Office. Then it is sent to Georgia Department of Behavioral Health and Developmental Disabilities main office for funding approval. If the individual is approved for a Medicaid Waiver, he or she will be assigned the Planning List Navigator who will contact them and explain the waiting list and other information. A Support Coordinator will also be assigned. These individuals will assist the family in navigating services, including Host Home settings.
